In Table 1, the data for patient no. 2 was inadvertently attributed to patient no. 4; the data for patient no. 3 was inadvertently attributed to patient no. 2; and the data for patient no. 4 was inadvertently attributed to patient no. 3.
In Figure 3, Lenalidomide was inadvertently misspelled as Lanalidomide.
